What you need to provide us:
- Business Name
- Business Contact -
Street / P.O. address, telephone and email - Evidence of Business Name Registration with the Arizona Secretary of State. ($10.00 fee is required by the Secretary’s Office for this registering your business name, which protects use of the name).

All sole proprietors must report business income to the Internal Revenue Service (IRS) using a social security number, or an Employee Identification Number (EIN) and all employees of the business must have an EIN.

The nutrient analysis vendors on this list represent available resources affiliated with the Arizona Dietetic
Association that have expressed an interest in working with the Arizona Smart Choice Program. The Arizona
Department of Health Services does not mandate the use of these vendors and recognizes that other reputable nutrient
analysis companies can provide such services provided that they have relevant expertise and experience and are an
established business with the State of Arizona.
Information to obtain from prospective nutrition professionals that can help you in your selection includes the following:
Expertise: Degree in Dietetics and/or Nutrition (at least an Associates Degree)
Experience: Experience providing nutrient analysis services
Established Business: Evidence of Business Name Registration with the Arizona Secretary of State
Culinary Skill: Experience cooking from standardized recipes
